(Customers purchasing plans for personal use)
Eligibility
OSU Wireless-Cellular service is available to all current Ohio State University faculty and staff for business use. Service ordered by departments for employees is intended for university business only. If you leave the university for any reason, you will no longer be eligible for this service and you are responsible for returning the wireless phone and any accessories to the department that ordered service. The department ordering this service will remain responsible for any and all charges incurred. OSU Wireless-Cellular cannot be transferred or shared with anyone else. Additionally, you must be 18 years or older to order and use OSU Wireless-Cellular service.
Rates and Charges
You agree to pay all applicable charges during the minimum term. Such charges include, but are not limited to: activation fees, monthly access charges, feature charges, airtime charges, toll charges, roaming charges, taxes, cancellation charges, and any other applicable charges as may apply. Different service rates and fees may apply when roaming outside of your service plan's area and on another carrier's network.
Service Changes
OSU Wireless-Cellular can change, modify or amend this agreement at any time by sending you written notice. If you do not agree with the changes and wish to cancel service, you must notify us in writing within 30 days of receiving any written notification and pay any outstanding charges. Cancellation fees and other charges may apply.
Suspension and Termination of Service
OIT reserves the rights to suspend or terminate service without prior written notice.
Changing Service Plans
There will be no charge for a first-time plan change. Additional plan changes will be subject to a $35.00 order charge.
Numbers
Your OSU Wireless-Cellular telephone number is assigned by OIT and The Ohio State University; you shall not acquire any proprietary interest in this number.
Warranty on Equipment
The warranty period for any cellular telephone purchased or provided through OSU Wireless-Cellular is subject to the terms and conditions of the manufacturer. The warranty is not transferable.
Services
Neither the supplier of services to OSU Wireless-Cellular, nor OSU Wireless-Cellular shall have any liability to any failure or disruption of services provided hereunder, regardless of the form of action, whether in contract or in tort or otherwise. You shall not be deemed a third-party beneficiary of any contract between OSU Wireless-Cellular and OSU Wireless-Cellular supplier.
Theft and Unauthorized Use of Service and Equipment
You must make every effort to safeguard your OSU Wireless-Cellular service and phone from unauthorized use. You are responsible for notifying OIT to deactivate your service immediately if you suspect misuse, or if your phone is lost or stolen. If fraudulent use is suspected by OIT or The Ohio State University, this service may be suspended or terminated at any time. In the event that your phone is lost or stolen, you are responsible for the terms and conditions of this agreement, as well as the purchase price of a replacement phone. Term
The term of this agreement is effective upon activation of the cellular telephone number(s) listed on page one (1) of your OSU Wireless-Cellular Personal Agreement. As the subscriber, you agree to a minimum period of two (2) years. This agreement will remain in effect for the duration of the minimum term. Periods of suspension of service do not count towards your minimum term.
Service Changes

If you do not agree with the changes and wish to cancel service, you must notify our office in writing within 30 days of receiving any written notification. Any service changes you request including service termination must be made to OIT in writing via a 100W or internal microform. No verbal change requests will be accepted.
